郭艺芳:检测微量白蛋白尿真的应该退出历史舞台了吗?

2014年美国糖尿病协会(ADA)年会期间,美国哈佛大学著名学者Krolewski指出:“微量白蛋白尿(MAU)不能作为早期、进行性肾功能下 降的指标,到了用其他指标替代的时候了”,其理由是部分1期和2期慢性肾病患者并不能检测出MAU,这使得不伴MAU的早期肾功能下降患者或有可能发展为 终末期肾病的患者不能得到早期干预和治疗。基于Krolewski教授的观点,国内外很多学者认为通过检测MAU评

2014年美国糖尿病协会(ADA)年会期间,美国哈佛大学著名学者Krolewski指出:“微量白蛋白尿(MAU)不能作为早期、进行性肾功能下 降的指标,到了用其他指标替代的时候了”,其理由是部分1期和2期慢性肾病患者并不能检测出MAU,这使得不伴MAU的早期肾功能下降患者或有可能发展为 终末期肾病的患者不能得到早期干预和治疗。基于Krolewski教授的观点,国内外很多学者认为通过检测MAU评估早期肾功能损害并非理想的手段,故此 方法应该被淘汰。

这一观点有失偏颇,理由如下:

1. 目前尚缺乏更为可靠的新指标取代MAU检测。通过检测MAU固然有其局限性,确有部分早期肾脏损害患者尿蛋白排泌率并无明显增高。然而,迄今应用于临床的各项技术中,尚缺少更为简便、经济、有效的方法取代MAU,因此在可预见的将来,MAU仍将是一种实用且效价比较好的技术手段;

2. MAU所反映的不仅是早期肾脏损害。肾小球毛细小管内皮损伤可导致白蛋白从血液中渗漏出进而 进入尿液。肾小球毛细血管的这种结构特点与全身血管相似。MAU的出现不仅反映了肾脏的病变,同时也反映全身血管内皮的损害,后者则是动脉粥样硬化病变发 生发展的核心机制之一,因而MAU的存在常常提示动脉粥样硬化性心血管疾病的病理生理过程已经启动。大量临床研究证实,与无MAU的患者相比,伴MAU的 高血压和(或)糖尿病患者发生颈动脉内膜增厚、左心室肥厚、缺血性心脏事件以及外周血管疾病的风险显著增加,因而MAU也是高血压糖尿病患者发生心血管 事件的重要危险因素。MAU的检测简便易行,有助于及早发现早期肾脏损害与心血管高危人群,因此应成为心血管高危患者风险评估时的一项重要指标;

3. 现有研究证实以MAU为靶点进行干预是有益的。近年来一系列研究显示,积极治疗MAU有助于 改善患者心血管与肾脏预后。LIFE研究显示,在基线UACR增高的患者中,应用氯沙坦治疗1年后UACR降低者较UACR持续增高者主要复合终点事件 (心血管性死亡,非致死性卒中心肌梗死)发生率显著下降,提示通过合理的治疗减少MAU有助于显著降低高血压患者心血管事件风险。IRMA-2研究显 示,厄贝沙坦治疗后受试者尿白蛋白排泄量显著降低。与此同时,厄贝沙坦300mg/d组2年后发生糖尿病肾病的相对风险较安慰剂组降低了70%。在医疗经 济学方面,积极筛查与干预高血压患者的早期肾脏损害具有良好的效价比。一项以IDNT研究数据为基础的分析显示,在高血压糖尿病患者中积极筛查MAU并应 用厄贝沙坦进行干预,可以有效减少终末期肾病的发生进而节省医疗费用。筛查的时机越早,其获益幅度越大。STENO-2研究也显示,为伴MAU的糖尿病患 者进行降糖治疗并应用RAS抑制剂、阿司匹林及降脂药物,可以有效降低患者不良心血管事件和肾脏事件发生率。上述研究表明,积极监测与合理干预MAU对降 低高血压和糖尿病患者心、肾、血管等并发症发生率具有积极意义。

鉴于上述考虑,目前不应否定MAU的临床价值。对于高血压和(或)糖尿病患者,应将MAU检测作为临床常规的检查内容 之一。对于伴MAU的高血压和糖尿病患者,既要强调血压和(或)血糖达标,也要强调尿蛋白排泄量的达标。通过综合治疗措施将MAU降到尽可能低的水平,有 助于降低不良心血管与肾脏事件的发生率。
