European Radiology:可预测卒中后癫痫的新型放射组学模型!

卒中后癫痫(PSE)是卒中的常见后遗症,临床上定义为卒中后7天以上的癫痫发作,影响3.1-9.5%的卒中患者。癫痫病妨碍卒中后的恢复,并导致进一步的神经功能缺损造成更高的住院费用预期寿命缩短以及癫痫相关的死亡率升高。出血性卒中,尤其是脑内出血(ICH),与PSE相关的风险比脑梗塞高近10倍8年累计发生率近15%。

相当多的研究调查了出血性卒中后PSE的预测因素。这些研究发现,早期癫痫发作(定义为卒中后7天内发生的癫痫)、指数卒中时年龄较小、ICH体积较大、皮质受累、存在脑室内出血是ICH后晚期发作的重要预测因素有关。然而,这些研究仅限于分析传统的临床变量和传统的神经影像学数据。

2012年,Philippe Lambin等人首次描述并提出了放射组学概念,可以从计算机断层扫描(CT)、正电子发射断层扫描(PET)或磁共振成像(MRI)获得的放射图像中以高通量提取和分析大量高级定量成像特征。作为一种高度自动化的计算方法,放射组学的效用已在许多领域得到证实,如预测早期癌症的淋巴结转移胶质瘤患者术前MRI图像的术后癫痫发作风险分析评估最终后认知障碍的概率以及预测ICH后的血肿扩张。然而,放射组学在预测PSE方面的应用还没有被研究。


近日,发表在European Radiology杂志的一项研究探讨了放射组学在预测与ICH相关的PSE中的作用及价值,同时建立了一个放射组学与临床相结合的模型并评估了其与单独临床模型性能之间的差异及优势。

本项研究建立了由2010年至2020年间在中国两家三甲医院就诊的首发ICH患者所组成的推导和验证队列。临床数据从电子病历中提取,并通过电话访谈进行补充。通过多变量逻辑回归选择预测性临床变量建立临床模型。识别出预测性的放射组学特征,并根据所选特征的系数计算出Rad-score。将临床变量和放射组学特征结合起来建立放射组学-临床模型比较了临床、Rad-score和组合模型的性能。 

共有1571名患者被纳入分析。临床模型中包括皮质受累、ICH后7天内的早期癫痫发作、NIHSS评分和ICH体积。联合模型中包括Rad-score,而不是ICH体积。在决策曲线分析(DCA)中,联合模型表现出更好的鉴别能力,并在阈值概率方面取得了比临床模型更好的总体效益。

本项研究表明,与临床模型相比,放射组学-临床联合模型能够更好地预测ICH相关的PSE。这可以协助临床医生更好地预测患者在首次ICH后发生PSE的风险,并促进PSE的早期诊断和治疗。
