JAMA:极早产儿氧饱和度、死亡与严重致残风险

研究认为,对于极早产儿,不同的氧补充策略对患儿18-24个月后死亡以及严重致残率的影响不显著,低氧补充治疗患儿死亡以及坏死性小肠结肠炎风险增加,但高氧补充患儿的视网膜病变风险增加

近日研究人员就极早产儿血氧饱和度和氧补充对新生儿死亡以及严重致残的影响进行了考察。

月龄不足28周的极早产儿参与研究,通过脉搏血氧测定法(SpO 2)评估新生儿的血氧饱和度,采取Spo2 85-89%或91-95%二种不同的氧补充方案,主要终点为死亡和严重致残(双盲、耳聋、总体运动功能分类系统上被诊断为≥2级的脑性瘫痪以及Bayley-III认知或语言评分<85)的复合终点,研究同时设定了16项次要终点。

4965名新生儿参与研究,其中2480人接受低氧补充方案,2485人接受高氧补充方案,参与者平均月龄26周,出生体重832g。低氧补充组患儿中1191人(53.5%)以及高氧补充组患儿中1150人(51.6%)出现主要终点事件,差异不显著。次要终点中,11项组间无差异,2项低氧组更佳,3项高氧组占优。低氧组484名(19.9%)患儿以及高氧组418名患儿(17.1%)死亡。低氧组220名(10.9%)患儿以及高氧组308名患儿(14.9%)接受早产儿视网膜病变治疗。低氧组227名(9.2%)患儿以及高氧组170名患儿(6.9%)出现坏死性小肠结肠炎。

研究认为,对于极早产儿,不同的氧补充策略对患儿18-24个月后死亡以及严重致残率的影响不显著,低氧补充治疗患儿死亡以及坏死性小肠结肠炎风险增加,但高氧补充患儿的视网膜病变风险增加。
