“当归-益母草”药对:治疗流产后并发症的机制研究

“当归-益母草”药对在流产并发症的治疗中起着重要作用,其作用机制是什么?

      人工流产是终止妊娠的一种普遍方法,20世纪70年代初前列腺素的出现,使得药物流产成为终止早孕的一种选择。自从抗孕酮药物米非司酮(RU486)于2000年在美国登记用于流产以来,药物流产由于其相对安全和方便,在早期妊娠中逐渐替代真空抽吸而被用于门诊。虽然人工流产的安全性较以前有所提高,但仍存在许多并发症,如免疫障碍、凝血障碍、盆腔感染、贫血,甚至女性不孕症。因此,寻找有效治疗药流并发症的药物十分重要。

     “当归-益母草”最初源自《竹林寺女科密方》记载的益母丸,为治疗妇科疾病的传统古方,具有补益气血,祛瘀生新的作用。《本草纲目》云:“益母草行血养血,行血而不伤新血,养血而不滞瘀血,诚为血家之圣药也。”被认为是治疗月经不调、胎漏难产、产后恶露不尽、瘀血腹痛等妇科常用良药。当归被称为“补血要药”,其补血作用得到历代医家公认,具有补血活血、调经止痛的功能。

     “当归-益母草”药对在药流并发症的治疗中起着重要作用,但其作用机制尚不清楚。近期在Journal of Ethnopharmacology发表一项研究,通过网络药理学筛选“当归-益母草”活性成分的相关靶点,并通过药理学实验来验证其可能的治疗机制

      网络药理学分析结果显示,调节T淋巴细胞及相关细胞因子可能是“当归-益母草”在贫血、盆腔感染、免疫疾病、凝血疾病等流产后并发症治疗中发挥作用的主要机制之一。药理学结果表明,“当归-益母草”极大程度纠正了RU486流产小鼠Th细胞亚群的失衡,显著抑制了Th2细胞的分化,导致Th1/Th2比值升高。此外,流产后小鼠血清中孕酮的浓度也明显降低。“当归-益母草”上调小鼠脾脏T-bet,下调IL-4基因表达。

      结果表明,“当归-益母草”作为一种替代免疫调节剂有可能应用于临床,用于治疗因流产引起的免疫失调,使其更快地恢复正常。
