高催乳素血症诊断和治疗通常受到催乳素,大分子催乳素的生物活性和临床上不相关的高分子量复合物的存在的干扰。本研究的目的是通过常用的自动免疫测定平台来评估两种大催乳素筛选方案的性能。

研究人员使用六种免疫测定方法对来自健康供体的女性(n = 96)和男性血清(n = 127),进行参数总体和单体性别特异性参考区间的划定。参考区间使用27种高催乳素血症和大催乳素血清进行验证,采用凝胶过滤色谱(GFC)测定催乳素的单体和体。

六种催乳激素测定的规范数据包括一定的范围值(2.597.5百分位数)。采用单体参考间隔法的95%置信区间]与聚乙二醇(PEG)回收截断法(平均值= 1.8; 95CI 0.8-2.8)相比,验证血清(高泌乳素和催乳素; n = 27)显示较高的不一致分类[me​​an = 2.8; CI1.2-4.4]。两种单体/大分子催乳素的鉴别方法没有显显差异(p = 0.089)。在通过两种鉴别方法评估的大肠上皮性血清中,CobasArchitect / Kryptor催乳素分析分别是最低和最高的错误分类数。

目前用于催乳素测试的自动化免疫测定需要基于PEG沉淀的大分子催乳素筛选方法,以便区分真正升高的血清催乳素。虽然回收截断法和单体参考区间大催乳素血清筛选方法表现出类似的鉴别性能,但是后一种方法还能给临床医生提供易于解释的单体催乳素浓度以及单体参考区间。
